About The Position

The Nurse Navigator at Elmwood Primary Care serves as a liaison among patients, professional staff, and physicians, providing care management and coordination for adult and pediatric patients with complex conditions, social needs, or educational needs. The role collaborates with primary care providers, specialists, other health care team members, and patients and families to support quality, efficient, and cost-effective care. The Nurse Navigator assesses, plans, implements, coordinates, monitors, and evaluates available options and services to optimize patients’ health status and reduce hospital and emergency room utilization. The position uses evidence-based clinical guidelines and protocols to develop individualized, patient-centered care plans that promote quality and efficiency. The navigator is expected to work in an organized and time-conscious manner and consistently use effective customer service and interpersonal skills.

Requirements

Currently licensed as a Registered Nurse in the State of Michigan.

Nice To Haves

Bachelor of Science degree in nursing preferred.
Minimum 5 years of experience as an RN is highly preferred.
