The Center for Women’s Health and Wellness located at 67 Corporate Drive in Portsmouth, NH offers comprehensive, personalized care for all stages of a woman's life in an atmosphere of privacy and trust. Our team of expert providers and specialists at OB/GYN and Infertility take a thoughtful approach to provide high-quality compassionate care in the areas of Obstetrics, Gynecology, Urogynecology, Infertility and LGBTQ+ care. We are seeking a full-time, 37-hour Urogynecology Nurse Navigator. The schedule will consist of three 9-hour days and one 10-hour day per week, with Wednesdays off. The position includes one remote workday per week. The Urogynecology Nurse Navigator serves as a specialized clinical resource and patient advocate for women experiencing pelvic floor disorders, including urinary incontinence, pelvic organ prolapse, recurrent urinary tract infections, voiding dysfunction, and other urogynecologic conditions. The Nurse Navigator provides comprehensive care coordination, patient education, clinical triage, and procedural support while guiding patients through evaluation, treatment, and recovery. Working collaboratively with physicians, advanced practice providers, nursing staff, and ancillary services, the Nurse Navigator ensures seamless, patient-centered care across the continuum, helping patients navigate complex treatment plans while improving access, outcomes, and patient satisfaction. This role works closely with the urogynecology provider to support clinical operations and optimize patient care delivery.

Requirements

  • Bachelor's Degree Nursing required
  • Registered Nurse [RN - State License] - preferred
  • Basic Life Support [BLS Certification] - preferred
  • Experience in an acute care setting and/or provider practice and/or experience in a community-based role or with discharge/transitional planning background 2-3 years required
  • Strong attention to detail and organization.
  • Strong cultural awareness competencies.
  • Proficiency in Microsoft Office.
  • Maintains good communication skills.
  • Readily adapts to change and welcomes new approaches when circumstances demand it.

Nice To Haves

  • Women's Health, OB/GYN, Urology, Urogynecology, Surgical Services, Ambulatory Care, or Care Coordination experience.
  • Experience managing complex patient populations and coordinating multidisciplinary care.
  • Strong triage, patient education, and care management skills.
  • Experience with prior authorizations, surgical coordination, and electronic medical records.
  • Demonstrated ability to work independently while maintaining strong collaboration with providers and care teams.

Responsibilities

  • Serve as the primary clinical point of contact for urogynecology patients throughout their care journey.
  • Provide telephone and electronic triage, clinical assessment, and symptom management within scope and established protocols.
  • Coordinate care for patients undergoing diagnostic testing, procedures, and surgical interventions.
  • Deliver individualized patient education regarding pelvic floor disorders, treatment options, medications, catheter management, pessary care, and post-operative recovery.
  • Support in-office urogynecology procedures, including bladder instillations, urodynamic testing, cystometrics, post-void residual assessments, bladder irrigation, catheter teaching, and pessary management.
  • Facilitate prior authorizations, appeals, referrals, and coordination with external providers and community resources.
  • Monitor patient progress and proactively identify barriers to care, ensuring timely intervention and follow-up.
  • Collaborate closely with providers to optimize patient outcomes and enhance continuity of care.
  • Participate in quality improvement initiatives and departmental projects designed to improve patient experience and operational efficiency.
  • Responsible for coordinating the complex care needs of patients and is the primary patient contact for service access, diagnostics, treatment activities, schedules, patient and family education, and multidisciplinary collaboration.
