Requirements

• Graduate of an Accredited Program in Nursing
• RN - Registered Nurse with Current Ohio License
• 3-5 years Nursing experience
• BSN required within 3 years of hire (for new hires effective 4/1/2019)

Nice To Haves

• BSN (Preferred Education)
• Specialty certification aligned with assigned patient population - required within 2 years of hire
• 3-5 years experience in acute patient care and/or case management.
• 1-2 years experience in assigned area.

Responsibilities

• Works collaboratively with physicians and the health care team to guide patients through the often difficult process of clinical management for an assigned patient population.
• Serves as an integral part of the health care team, ensuring safety, best practice, and high-quality standards of care across the health care continuum.
• Acts as an advocate, interpreter, educator, and counselor for assigned patients.
• Coordinates, assesses, plans, and implements patient care throughout the continuum of care in collaboration with a multidisciplinary team for a unique patient population.
• Assists with reimbursement, allocation of resources, quality improvement, quality management, and utilization management.
• Provides professional nursing care, routine report out, and participates in all quality improvement initiatives.
• Serves as a liaison in the health care system and in the community.
