About The Position

The Nurse Navigator works collaboratively with physicians and the health care team to guide patients through the often difficult process of clinical management for an assigned patient population. This position is an integral part of the health care team who works to ensure safety, best practice, and high quality standards of care maintained across the health care continuum. The Nurse Navigator is an advocate, interpreter, educator and counselor for assigned patients. Nurse Navigator duties may include, but are not limited to coordinating, assessing, planning and implementing patient care throughout the continuum of care in collaboration with a multidisciplinary team for a unique patient population; assisting with reimbursement, allocation of resources, quality improvement, quality management and utilization management; and providing professional nursing care, routine report out and participation in all quality improvement initiatives. The Nurse Navigator serves as a liaison in the health care system and in the community.

Requirements

  • Graduate of an Accredited Program in Nursing
  • RN - Registered Nurse with Current Ohio License
  • 3-5 years Nursing experience

Nice To Haves

  • BSN required within 3 years of hire.
  • Bachelor's degree in nursing (BSN)
  • Specialty certification aligned with assigned patient population - required within 2 years of hire
  • 3-5 years experience in acute patient care and/or case management.
  • 1-2 years experience in assigned area.
