About The Position

A Non-merit vacancy exists in the Community Health Services/Community First Choice Program of the Baltimore County Department of Health and Human Services. Non-Merit positions are not classified within the Baltimore County Government Classification and Compensation Plan. All interested candidates must apply at this time. Nurse Monitors provide quality assurance oversight (QA) of Residential Service Agencies (RSAs) to ensure the health and safety of at-risk Medicaid participants. Nurse monitors liaison with Adult Evaluation and Referral (AERS) staff, Support Planners (SPs), and the Office of Health Care Quality (OHCQ) to facilitate the health and safety of participants in a variety of community settings. Monitors work flexible hours including evenings and weekends to meet the needs of participants who receive in-home services around the clock. Monitors possess medical-surgical/geriatric and/or pediatric experience, strong assessment skills, the ability to work independently in a home setting, and the capacity to instruct and evaluate providers of in-home assistance to vulnerable Medicaid participants across the age spectrum as needed. Monitors must be familiar with/able to learn and interpret applicable COMAR regulations. Monitors possess a high degree of electronic medical record experience/computer literacy and excellent documentation skills.

Requirements

  • Registered nurse with a Maryland license.
  • Minimum of 2 years professional nursing experience.
  • Must have at least 2 years medical/surgical experience.
  • Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment.
  • Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.
  • All licenses and certifications must be maintained at current levels throughout employment.
  • Knowledge of applicable COMAR regulations
  • Knowledge of nurse best practices for assessment and care planning
  • Knowledge of corrective action plans
  • Knowledge of nursing delegation/Nurse Practice Act
  • Must be able to learn and use current State Tracking System technology
  • Must be computer literate/EMR proficient
  • Must be able to navigate the County by car
  • Must be able to provide outreach and teaching (participants, families, SPs, RSAs)
  • Must be able to link participants to other services
  • Must be able to advocate for services as needed
  • Ability to work independently in a home setting
  • Ability to instruct and evaluate providers of in-home assistance to vulnerable Medicaid participants across the age spectrum
  • Ability to oversee/evaluate the performance of Residential Service Agencies (RSAs) to ensure the health and safety of at-risk participants
  • Applicants are required to submit proof of licenses, certifications and education beyond high school to meet the required and preferred qualifications of the position.

  • Applicants selected for employment with Baltimore County must successfully complete a physical examination, drug screening and employment investigation, including, but not limited to criminal background, education certification and fingerprinting checks.

Responsibilities

  • Review State Tracking System (LTSS) data/interRAI assessments and any attachments upon assignment of cases.
  • Contact assigned clients/representatives timely.
  • Make home visits at a frequency determined by the medical/social-emotional status of the participant.
  • Conduct in person Quality Assurance reviews annually and as needed.
  • Contact SPs and RSAs to coordinate care/resolve care issues promptly.
  • Verify presence and appropriateness of (nursing) care plans for participants.
  • Report deficiencies via LTSS to MHD and other agencies as needed.
  • Complete all documentation/submit all activity billing via LTSS timely and in keeping with Program policies/processes.
  • Make joint home visits with SPs and other agencies (i.e. Adult or Child Protective Services) as necessary.
  • Performs other related duties as required.
