About The Position

Unity Healthcare is the designated provider of Healthcare for the Homeless (HCH) in Washington D.C. To ensure this population has access to care despite daily challenges, Unity provides services at various sites across the city and through a continuum of care, including same-day walk-ins, primary care, specialty visits, and inpatient respite care for the medically vulnerable. The Nurse Manager for Healthcare for the Homeless (NM for HCH) oversees nursing staffing and systems across all three components of the HCH program: the CCNV Community Health Center, Homeless Outreach clinic sites, and Medical Respite sites.

Requirements

  • Current DC RN license
  • BLS or CPR certification
  • Bachelor’s degree in nursing
  • Master’s degree in nursing, Public Health, and/or other health related field
  • Minimum three years of experience as a manager and supervisor
  • Minimum 3 years of experience managing public health programs for vulnerable populations

  • 5+ years of clinical FQHC/Ambulatory care experience

Responsibilities

  • Staffing: Responsible for building a dynamic team of highly flexible, dedicated, autonomous, and competent nursing professionals (RNs, LPNs, MAs, CNAs) to meet all nursing staffing needs across the HCH program. In coordination with the HCH tripod, manages daily scheduling for CCNV, Homeless Outreach, and Respite, adjusting schedules and re-deploying team members as needed to meet program needs.
  • On-site coordination: Rotates through CCNV, Homeless Outreach sites, and Medical Respite sites to evaluate proper implementation, use, and coordination of practices and policies for all nursing staff.
  • Nursing systems: Designs, implements, and monitors systems necessary for the safe and efficient functioning of the program, including supply chain and storage for medical supplies, vaccines, and emergency equipment.
  • Environmental safety and quality: Maintains a physical environment of care and a team culture that measures and promotes safety and quality, from hand hygiene to daily quality controls.
  • Compliance: Assures that all nursing services delivered across the HC program are provided in-line with Unity Healthcare Policies & Procedures, and in compliance with all applicable standards and requirements of all applicable regulatory bodies.
  • Program monitoring and evaluation: In partnership with the HCH tripod and Unity-wide quality improvement mechanisms, identifies, measures, and critically evaluates program process and outcomes measures on a periodic basis.
  • HCH program strategic governance: In close collaboration with the HCH tripod, and in close coordination with Unity executive leadership, helps provide strategic governance to the HCH program, including budget development and monitoring, crafting the strategic direction of the HCH program, and generally ensuring that the program meets both quality and sustainability goals.
  • Monitors Nursing: Provider ratios and ensures that program is appropriately staffed, working to fill vacancies and also avoiding staffing overages or inefficiencies.
  • Provides new team members with a comprehensive orientation, equipping them with the information they need to succeed in their role.
  • Observes team member strengths and weaknesses and provides targeted coaching, mentorship, and skills remediation as needed.
  • Establishes a format and rhythm for HCH nursing team meetings.
  • Evaluates annual performance for all direct reports, including establishment of individual and team goal metrics.
  • Collaborates with Health Center Directors, human resources, and medical/clinical leadership to effectively handle disciplinary action concerning clinical issues of nursing staff members.
  • Identifies and problem-solves performance issues within the team, including working with HR to develop and document performance improvement plans as needed.
  • Oversees medical supply ordering for the HCH program and monitors relevant expenses in line with the approved budget.
  • Oversees vaccine ordering, storage, and administration for the HCH program sites and monitors relevant budget expenses.
  • Oversees the completion of safety and environment of care audits for all HCH care delivery locations according to the number and rhythm defined by Unity nursing leadership.
  • Oversees the completion of chart audits for all roles supervised according to the number and rhythm defined by Unity nursing leadership.
  • In collaboration with nursing leadership, contributes to the development of standards and processes to support and evaluate nursing and medical assistant education, orientation, and competency assessment in the HCH context.
  • Contributes to Unity-wide quality improvement efforts and actively promotes a culture of continuous quality improvement.
  • Serves on committees as designated by the Vice President of Nursing.
  • Provides weekly and monthly reports to Unity executive leadership as requested.
  • Works closely with the HCH Tripod and Finance to develop and monitor an annual budget for HCH.
  • Works closely with the HCH Tripod to monitor and evaluate the performance of the different arms of the HCH program on key process and outcome quality indicators.
  • In collaboration with HCD, ensures that necessary contract services are in place for the safe functioning of all sites, including biohazard removal, lab specimen pickup, and oxygen delivery.
  • Collaborates with the HCH Tripod to organize seasonal vaccine outreach and disease-specific vaccine events as needed.
  • Works with Unity’s Grants Management team as needed, contributing to application for funding relevant to HCH and ensuring compliance with all grants that include an HCH component.
