As the HFA Nurse Manager, you’ll serve as a clinical resource for residents in Home for the Aged (HFA) and Independent Living (IL), while leading and supporting the care team to deliver exceptional, compassionate care every day. Requirements

  • Graduate of an accredited School of Nursing.
  • Valid RN or LPN license in the State of Michigan.
  • Minimum of 2 years of supervisory experience.
  • Previous clinical experience working in a licensed HFA community.
  • Current CPR and First Aid certification required.

Responsibilities

  • Provide supervision, guidance, and oversight of the Direct Care Supervisors and Caregiver Retention Specialist.
  • Provide clinical guidance to Resident Care Assistants and Medication Techs on the performance of their duties.
  • Assist Resident Care Assistants and Med Techs with job-related duties.
  • Function as a member of the HFA leadership team; work collaboratively to ensure the department is adequately staffed; and delegate/reassign team members to best meet resident needs.
  • Provide treatments, injections, or medications to residents as ordered by the physician.
  • Ensure documentation is informative, descriptive, and reflects the care provided to residents.
  • Communicate and collaborate with the interdisciplinary team to make decisions that ensure resident well-being is achieved.
  • Maintain and update Plans of Care (POC), coordinate ancillary services and referrals, and monitor clinical tracking processes (e.g., vitals, wound care, labs, TB testing).
  • Review and process physician notes, lab results, and diagnostic reports; ensure accurate and timely documentation.
  • Coordinate communication with PCPs and external partners.
