About The Position

Join The Commons on St. Anthony in Auburn, NY, a 297-bed facility offering opportunities in specialized nursing units, including Short-term Rehabilitation and Long-Term Care. The Nurse Manager conducts patient assessments, implements and evaluates the effectiveness of nursing care, and uses management and administrative skills to achieve corporate, department, and facility goals. This role reports to the Director of Nursing or similar and ensures adherence to established policies, procedures, and systemic clinical changes.

Requirements

3+ years’ experience in nursing, preferably in an acute/long term care setting.
Current NYS RN License in good standing required.
Proven patient assessment/evaluation skills.
Current knowledge of best practice nursing standards and care.
Proficient in the care planning process.
Ability and knowledge to respond in emergency situations.
Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
Ability to lead, guide and motivate staff.
Outstanding interpersonal skills and the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
Ability to read, write and speak clearly in English.

Nice To Haves

2+ years supervisory experience preferred

Responsibilities

Manages the clinical care planning process to ensure the coordination and delivery of quality patient care.
Reviews and updates the care plan accordingly.
Manages the delivery of patient care, maximizing quality and efficiency as well as applying best practice guidelines.
Provides or assists with direct care when needed, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated.
Communicates effectively with the client/resident, family and interdisciplinary team and ensures staff members communicate effectively as well.
Maintains the highest level of customer service within the area of responsibility.
Provides leadership and direction to personnel in accordance with organizational goals and mission.
Plans, implements, and evaluates the clinical competencies and on-going educational needs of the staff.
Mentors new employees and fosters positive, collaborative relationships.
Develops, reviews, and revises policies, procedures, and other applicable documentation to ensure regulatory agency standards are met.
Interprets policies, procedures, and regulations for the staff.
Ensures clinical staff understand and comply with all policies, procedures, and regulations.
Directs the processes that facilitate the thorough and accurate completion of required documentation. Examples include but are not limited to: skin/wound reports, ADLs, weights, nourishment, monthly MAR/TAR checks, admissions, discharges, incident reports, investigations, and audits.
Monitors adherence to patient care standards through Quality Assurance (QAPI/QA) processes.
Is actively involved in the development, implementation, and evaluation of QAPI activities.
Reviews Point of Right to make improvements and changes in quality measures and keeps Directors informed of patient care problems and issues in timely manner.
