About The Position

BAYADA Home Health Care is seeking a Registered Nurse (RN) to fill the position of Home Health Nurse Manager for our Williston, VT Medicare Certified Senior Living office. This office provides best-in-class care to adult and geriatric clients throughout Northern Vermont. We are looking for a compassionate, driven leader with a strong background in Medicare-certified home health, VNA services, and clinical team management who is passionate about developing clinicians and delivering exceptional patient outcomes.

Requirements

  • 2+ years of Home Health, Visiting Nurse Association (VNA), Hospice, or Medicare-certified home care experience required.
  • 2+ years of clinical leadership, management, supervisor, or nurse manager experience strongly preferred.
  • Strong understanding of Medicare regulations, OASIS, home health operations, and compliance standards.
  • Proven ability to lead, coach, mentor, and develop clinical teams.
  • Experience managing caseloads, care coordination, clinical performance, and quality outcomes.
  • Excellent organizational, communication, and relationship-building skills.
  • Passion for delivering exceptional patient care while leading others to success.

Responsibilities

  • Lead, mentor, inspire, and develop a team of Home Health clinicians to provide exceptional patient care.
  • Work closely with the Rehab Manager (where applicable), Director, and DCO to ensure cohesive and consistent management of all field staff.
  • Review and oversee all clinical documentation to ensure regulatory compliance and high-quality patient outcomes.
  • Partner with office leadership to ensure appropriate staffing, scheduling, and caseload management.
  • Ensure development, implementation, and ongoing updates to individualized patient care plans.
  • Coordinate client care, referrals, and act as an internal case manager and patient advocate.
  • Collaborate with physicians, community partners, senior living communities, and interdisciplinary teams to ensure continuity of care.
  • Participate in recruiting, interviewing, onboarding, and retaining top clinical talent.
  • Represent BAYADA in community outreach and marketing activities, including visits to physician offices, hospitals, senior living communities, and referral partners.
  • Drive quality outcomes, operational excellence, and staff engagement while fostering a positive team culture.
