About The Position

The Nurse Manager of Care Coordination manages daily unit operations including: staffing and scheduling; orientation and on-boarding; education and competency assessment; budget monitoring; maintaining and establishing patient care standards; and promoting evidence-based practice. The manager implements quality and safety initiatives, engages in process improvement, and is responsible for human resource management. This position applies standards for professional nursing practice and focuses clinical care to enhance the experience for patients, families, and all others while functioning in alignment with the leadership approach of St. Charles Health System. The Nurse Manager creates and sustains a patient care environment that supports safe, knowledgeable, compassionate, productive, and engaged caregivers. The Nurse Manager is a resource and support for the unit caregivers and departmental operations. This position directly manages caregivers in the Care Coordination department.

Requirements

  • Graduate of an accredited school of nursing.
  • BSN or bachelor’s degree in related field.
  • Current Oregon RN license.
  • Valid Oregon driver’s license and ability to meet St. Charles Health System driving requirements.
  • Ability to travel to business functions/trainings/meetings and all St. Charles Health System worksites.
  • AHA Basic Life Support for Healthcare Provider certification.
  • Minimum five (5) years of clinical nursing experience in an acute care setting.
  • Minimum one (1) year of leadership experience.
  • Knowledge of Quality Improvement.
  • Previous experience with emphasis on disease management, patient, and staff education.

Nice To Haves

  • Master’s degree in nursing, management, or related field.
  • Certification in Case Management from ACMA (CCM).
  • Three (3) years leadership or supervisory experience, preferably in a Utilization Review, Case Management, or Care Coordination acute-care setting.

Responsibilities

  • Responsible and accountable for successful daily operation of the Care Coordination department.
  • Participates in the development of operational plans, goals and performance targets for quality, customer service and regulatory compliance oversight to ensure principles and strategies of the organization are met.
  • Responsible for scheduling staff to provide adequate coverage and ensures adherence to staffing plan in accordance with par coverage requirements.
  • Maintains and demonstrates an in-depth knowledge of nursing principles, practices, standards, and techniques and applies this knowledge in accordance with St. Charles Health System policies and procedures and within laws and regulations governing nursing practice in the State of Oregon.
  • Maintains and demonstrates an in-depth knowledge of CMS Conditions of Participation for Discharge Planning and ACMA (American Case Management Association) practice standards.
  • Responds to patient and family concerns or complaints with sensitivity and compassion in a timely fashion.
  • Utilizes findings from patient satisfaction survey and with input from the caregivers, to develop department-based customer services goals.
  • Participates as member of healthcare team and various interdisciplinary committees to improve patient care and meet patient needs.
  • Promotes empowerment of staff by utilizing collaborative approaches and encouraging new ideas.
  • Responsible for budget development, regular monitoring, accountability and meeting all operational targets for all areas within span of control.
  • Hires, directs, coaches and monitors the performance of all direct reports, to develop and maintain a high-performance team that meets organizational and department goals.
  • Monitors and ensures all direct reports are current with compliance and safety requirements.
  • Implements and manages all organizational safety directives and goals.
  • Provides and oversees team’s delivery of customer service in a manner that promotes goodwill, is timely, efficient, and accurate.
  • Collaborates with teams to review processes and identify/implement opportunities for improvements, applying Lean principles, concepts and tools.
  • Creates a healing environment that supports all aspects of care, respects the wholeness of the individual, and demonstrates therapeutic presence through attitudes and behaviors that enhance the care experience for patients and caregivers.
  • Facilitate the growth and development of a culturally competent team in the provision of appropriate care to patients and family members who belong to diverse cultural backgrounds.
