About The Position

Benchmark Human Services is hiring a full-time nurse to coordinate healthcare and wellness for people served in its Group Home program. The role supports adults in small home settings as they build daily living skills and independence. The nurse works with individuals and their medical and interdisciplinary teams, provides advocacy, trains staff, coordinates medical care and transitions, and maintains health-related documentation. The position offers flexibility, continuity of care, autonomy, and the opportunity to build long-term relationships with individuals served.

Requirements

Current Indiana nursing license.
Valid driver’s license.
Experience working with individuals with intellectual or developmental disabilities and/or mental illness.

Responsibilities

Provide healthcare and wellness coordination in accordance with state and federal regulations.
Serve as a liaison and advocate for individuals’ healthcare with members of their medical and interdisciplinary teams.
Advocate for clients’ wellbeing and inclusion.
Train unlicensed staff on medication administration, delegated tasks, treatment protocols, and monitoring.
Implement and train employees on new and revised health-related policies and procedures.
Attend assigned medical appointments, as directed, and individual-specific, nursing, departmental, and other required meetings.
Manage transitions of care to and from hospitals, nursing homes, rehabilitation facilities, and other agencies.
Oversee pharmacy and medication needs.
Maintain health-related records and complete documentation and assessments.
Develop and update individual-specific health risk plans to ensure appropriate medical care.
Develop long-term relationships with individuals served.
Provide staff training to meet the healthcare needs of individuals served.
