About The Position

Benchmark Human Services is hiring an LPN Healthcare Coordinator for its Group Home program, which supports adults in developing daily living skills and independence in small home settings. The coordinator provides healthcare and wellness coordination, advocates for individuals’ wellbeing and inclusion, and works with medical and interdisciplinary teams. The role includes staff training, appointment participation, care transitions, medication and pharmacy oversight, health documentation, and development of individualized health risk plans. Benchmark emphasizes continuity of care, long-term relationships, and individualized care.

Requirements

Current Indiana nursing license.
Driver's license.
Experience working with individuals with intellectual or developmental disabilities and/or mental illness.

Nice To Haves

Experience in community or long-term nursing is preferred.

Responsibilities

Provide healthcare and wellness coordination in accordance with state and federal regulations.
Act as liaison and advocate for individuals’ healthcare with medical and interdisciplinary teams.
Advocate for clients’ wellbeing and inclusion.
Train unlicensed staff on medication administration, delegated tasks, treatment protocols, and monitoring.
Implement and train employees on new and revised health-related policies and procedures.
Attend assigned medical appointments and individual-specific, nursing, departmental, and other required meetings.
Manage transitions of care to and from hospitals, nursing homes, rehabilitation facilities, and other agencies.
Oversee pharmacy and medication needs.
Maintain health-related records and complete documentation and assessments.
Develop and update individual-specific health risk plans to ensure appropriate medical care.
Develop long-term relationships with individuals served.
Provide staff training to meet the healthcare needs of individuals served.
