About The Position
Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks a Per Diem Weekend Nurse Liaison to join its expanding weekend nurse team. This is an on-site position located at Newton Wellesley Hospital.
Requirements
- Licensed by the Commonwealth of MA as a Registered Nurse.
- Excellent communication and interpersonal skills
- Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families
- Highly skilled at providing patient assessments and evaluations.
- Excellent problem-solving skills
- Competency with accepted standards of hospice nursing practice.
Nice To Haves
- At least two years experience preferred in community health, oncology, hospice, or palliative care.
Responsibilities
- Provide information about hospice services to patients, families, and hospital-based staff.
- Conduct clinical assessments.
- Coordinate hospital discharge plans.
- Maintain professional relationships within hospitals, with a focus on the assigned hospital(s).
