About The Position

The Community Health Education Nurse supports the hospital’s commitment to improving community health through education, chronic disease prevention and management, wellness, outreach, and collaborative partnerships.

Requirements

  • Graduate of an accredited school of nursing with an Associate Degree in Nursing required.
  • Current Registered Nurse license or privilege to practice in Texas.
  • Current driver’s license
  • BLS certification

Nice To Haves

  • Bachelor of Science in Nursing or advanced nursing degree preferred.
  • Related certification, such as CHES or Chronic Care Professional, preferred.
  • Community health, public health, health promotion, population health, or health education experience preferred.
  • Knowledge of health promotion, chronic disease prevention and management, wellness, population health, health literacy, and barriers to care.
  • Strong teaching, presentation, facilitation, written communication, and public-speaking skills, with the ability to translate clinical information for diverse audiences.
  • Demonstrated relationship-building, program planning, project management, organization, data evaluation, and reporting skills.
  • Ability to work independently and collaboratively across hospital and community settings while demonstrating initiative, adaptability, professionalism, and follow-through.

Responsibilities

  • Develops, coordinates, delivers, and evaluates evidence-based community education, chronic disease prevention and management, wellness, and outreach initiatives.
  • Uses CHNA findings, health data, community input, and hospital priorities to translate identified needs into practical programs.
  • Serves as a primary hospital liaison for community health initiatives and connects partners with appropriate hospital leaders and subject-matter experts to improve access, prevention, chronic disease outcomes, and overall community health.
  • Develops and delivers evidence-based education on chronic disease prevention and management, including risk-factor reduction, symptom awareness, healthy lifestyles, treatment adherence, and appropriate use of healthcare resources.
  • Provides classes, workshops, presentations, screenings, vaccination clinics, health fairs, support programs, and other outreach within nursing scope, hospital policy, and regulatory requirements.
  • Collaborates with clinical and community partners to connect individuals with screenings, preventive services, chronic disease management resources, and ongoing care when needs are identified.
  • Adapts education to the health literacy, cultural, demographic, and clinical needs of diverse populations.
  • Identifies and engages populations experiencing barriers to health, healthcare access, or preventive services.
  • Uses CHNA findings, health data, community feedback, partner input, and hospital priorities to identify and respond to community health needs.
  • Develops action plans with defined target populations, goals, interventions, timelines, partners, resources, and measures of success.
  • Plans and coordinates community programs, events, conferences, and special initiatives, including content, speakers, logistics, communications, and materials.
  • Collaborates with clinical departments, physicians, nursing, marketing, quality, population health, vendors, and subject-matter experts to deliver effective community-facing programs.
  • Serves as a liaison for community health initiatives and maintains relationships with organizations and stakeholders aligned with identified priorities.
  • Identifies and engages partners based on shared goals, target populations, service gaps, and opportunities to improve health.
  • Represents the hospital in community meetings, coalitions, committees, and collaborative initiatives as assigned.
  • Coordinates shared objectives, roles, resources, and follow-through; connects partners with appropriate hospital leaders and experts.
  • Establishes process and outcome measures for programs and partnerships, including reach, participation, services delivered, engagement, and completion of commitments.
  • Maintains accurate documentation and evaluates effectiveness, resource use, service gaps, and progress toward intended outcomes.
  • Uses findings to improve initiatives and demonstrates changes in knowledge, behavior, access, prevention, or other appropriate outcomes.
  • Prepares concise reports and presentations communicating results and community impact to leaders and stakeholders.
