University of Iowa Health Care Department of Nursing is seeking a Nurse Clinician-Discharge Coordinator (PICU) to perform specific tasks to assist members of the Nursing team in providing patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division. The Nurse Clinician-Discharge Coordinator for the Pediatric Intensive Care Unit at University of Iowa Stead Family Children’s hospital functions as a clinical expert and care coordinator for pediatric patients with special and complex healthcare needs. The discharge coordinator manages care for the identified specific ICU patient population including IMCU (Intermediate Care Unit) and follows these patients through the continuum of care. The discharge nurse optimizes utilization of resources and service delivery including promoting patient throughput. They provide ongoing support and expertise through comprehensive assessments, discharge care planning, plan implementation and overall evaluation of patient’s needs for the next level of care. They work closely with the interdisciplinary team to enhance communication through daily bed huddles and rounding between team members, patients and their families.
