- Registered Nurse: MN

- Experience as a Discharge Coordination and/or in Case Management
Responsibilities
- Facilitates coordination of a multidisciplinary plan of care to guide patient progress to optimal care transitions
- Performs evaluation of patient’s care and progress for the episode of care and/or across the continuum of care for the identified population
- Reviews patient medical records to determine justification of admission, treatment and continued stay criteria
- Determines and facilitates the appropriate post-hospital discharge destination for a patient
- Identifies patient needs, capacity and preferences to determine what is required for a smooth and safe transition to patient’s discharge destination
- Actively involves patient, or individual acting on patient’s behalf, in planning for discharge
