The Nurse Case Manager will provide support to patients and healthcare providers by offering information on Patient Support services, specific diseases, and therapies. This role involves assisting patients and caregivers with assessments, acting as a liaison for medication access, and addressing general inquiries. The Nurse Case Manager will offer continuous support to ensure patient adherence, refer patients to healthcare providers for clinical questions beyond program materials, and document patient information in the electronic patient record. Responsibilities include ongoing file review, following case-management protocols, collaborating with reimbursement specialists and physicians to secure funding, and ensuring patient services are coordinated for expedited access to therapy. The role also involves participating in specialized training, attending meetings, coordinating with healthcare providers and pharmacies, maintaining communication with management, and assisting in training new Nurse Case Managers. Reporting Adverse Events and other assigned duties are also part of the role, with potential standby/on-call requirements.
