Compass Nurse Case Manager
About The Position
This is a part-time opportunity for a compassionate healthcare professional to join Seniority Healthcare as a COMPASS Nurse in the Long Island/Yaphank area. The COMPASS program (Collaborative Outpatient Management for Palliative and Aging Support Services) focuses on providing in-home nursing and case management services to geriatric patients, aiming to make a significant impact on their lives. Seniority Healthcare, founded in 2016, is dedicated to improving care for seniors and those with complex health needs through a model that combines frequent primary care, telehealth, and psychiatric support to reduce hospitalizations and provide comfort. They serve patients across NY, NJ, PA, and DE and value their team's well-being.
Requirements
- Minimum of 2 years of experience in a similar role, working with geriatric populations.
- LPN or RN license.
- Proficiency in EHR systems.
- Strong communication skills and ability to work effectively in a team environment and remotely.
- Phlebotomy skills.
- LPN or RN (Required)

- In-depth knowledge of health care services and medical terminology preferred.
Responsibilities
- Communicate effectively with patients, families, providers, and community staff regarding all aspects of patient care, including updates on treatment plans, concerns, or changes in condition.
- Manage and coordinate care for patients, documenting and assisting with orders for testing, medications, and referrals.
- Upload and document all health information into the patient's chart accurately and timely.
- Obtain vital signs and assist providers as needed during patient visits.
- Conduct medication reconciliation and maintain a professional relationship with the PCP.
- Coordinate referrals, therapy orders, and telemedicine appointments.
- Alert providers of acute issues.
- Check for patient updates and communicate with case management departments.
- Maintain HIPAA compliance and communicate effectively with other members of the care team.
