About The Position

We are seeking a Nurse (RN) Case Manager to join our team at University Hospital in Madison, WI. This role is 100% FTE, day shift, Monday - Friday, with 3-4 weekends per year and a holiday rotation. You will bring a strong clinical background, case management experience, and well-developed assessment and critical thinking skills. Your responsibilities will include assisting with anticipating the discharge needs of post-surgical patients to ensure timely discharge and access to the best possible resources. You will need an understanding of medical necessity, level of care, and patient status (inpatient vs outpatient), as well as knowledge of community resources such as transportation, skilled nursing facilities, or home health agencies. The ideal candidate will enjoy learning, asking questions, and working within a collaborative team environment.

Requirements

  • Strong clinical background.
  • Case management experience.
  • Well-developed assessment and critical thinking skills.
  • Bachelor's Degree in Nursing. Equivalent combination of education and experience will be considered.
  • 3 years of relevant clinical nursing experience.
  • Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Upon Hire.

Nice To Haves

  • Master’s degree in Nursing or healthcare related field.
  • Recent experience as a hospital-based Nurse Case Manager.
  • ACMA certification as a Case Manager.
