About The Position

Optum Home & Community Care’s Senior Community Care team is hiring an Advanced Practice Clinician (APC) for its SNF Patient Connect Program in greater Central Hamilton County, Ohio. The program provides a unique, one-time visit for UnitedHealthcare members transitioning from a hospital to a skilled nursing facility and ultimately back home. Its goals include improving care transitions, reducing 30-day hospital readmissions, and enhancing patient outcomes. Working autonomously and in collaboration with the case management team, the APC conducts comprehensive, one-time health assessments and documents acute and chronic conditions, diagnoses, and quality measures. The visit includes an initial medication review and support for Advanced Care Planning directives. The APC educates patients and families and communicates pertinent findings and medical concerns to attending providers, facility staff, and other necessary team members. The role does not involve ongoing care management or responsibility for orders, treatments, or follow-up. The position involves travel between care sites and work across varied care settings. It requires at least 10 hours per week, attendance at mandatory educational and computer/virtual training, and entering available hours in a web-based application at least one month before the available work time. The APC is also expected to check Optum email intermittently for training, communications, and monthly scheduling.
