About The Position
Reid Health is seeking a patient-centered and compassionate NP or PA to join our PACE (Program of All-Inclusive Care for the Elderly) team. This APP will provide primary medical management for PACE participants in collaboration with our PACE physicians and interdisciplinary care team, with a focus on comprehensive assessments, ongoing care planning, and supporting the health and well-being of our participants.
Requirements
- Successful completion from a qualified NP/PA program
- Indiana Professional License
- Must maintain BLS/ACLS certifications
Responsibilities
- Provide primary medical management for PACE participants in collaboration with PACE physicians and the interdisciplinary care team.
- Conduct comprehensive assessments.
- Develop and implement ongoing care plans.
- Support the health and well-being of PACE participants.
