Requirements

• Bachelor of Science Degree in Nursing and 4 years of clinical experience OR Associate Degree in Nursing with 10 years of nursing experience
• Excellent communication and listening skills.
• Demonstrated assertiveness, coupled with tact and diplomacy.
• Ability to relate well with hospital managers, physicians, and office staff.
• Technical skill in PC based computer systems.

Responsibilities

• Serves as a core clinical resource within the Connect Call Center.
• Acts as a critical access point for Northside Hospital clientele by ascertaining their needs and seamlessly connecting callers to appropriate hospital providers, clinics and resources.
• Utilizes strong clinical judgment and compassionate communication to ensure patients receive timely guidance and access to care that aligns with their health care priorities.
• Manages over 10,000 monthly calls and serves as the Frontline for key interdisciplinary teams within the organization.
• Demonstrates exceptional critical thinking, active listening and empathy during the call ensuring each patient feels heard, supported and safely directed to the care they need.
• Plays a key role in post-discharge care coordination through structured follow-up calls to evaluate patient recovery progress, identify early signs of complications, reinforce discharge instructions, and remove barriers that could lead to hospital readmission.
• Supports safe transitions of care, enhances patient satisfaction and contributes to improved clinical outcomes across the health system by delivering proactive outreach and education.
