About The Position

The Northside Connect Nurse serves as a compassionate and highly competent liaison and a core clinical resource within the Connect Call Center. This nurse acts as a critical access point for Northside Hospital clientele by assessing their needs and connecting them to appropriate hospital providers, clinics, and resources. Utilizing strong clinical judgment and compassionate communication, the nurse ensures patients receive timely guidance and access to care that aligns with their healthcare priorities. This role manages over 10,000 monthly calls, serving as the frontline for key interdisciplinary teams. The nurse demonstrates exceptional critical thinking, active listening, and empathy to ensure patients feel heard, supported, and safely directed. Additionally, this nurse plays a key role in post-discharge care coordination by conducting structured follow-up calls to evaluate recovery progress, identify early signs of complications, reinforce discharge instructions, and remove barriers to prevent hospital readmission. Through proactive outreach and education, the nurse supports safe transitions of care, enhances patient satisfaction, and contributes to improved clinical outcomes.

Requirements

  • Bachelor of Science Degree in Nursing and 4 years of clinical experience OR Associate Degree in Nursing with 10 years of nursing experience
  • Excellent communication and listening skills.
  • Demonstrated assertiveness, coupled with tact and diplomacy.
  • Ability to relate well with hospital managers, physicians, and office staff.
  • Technical skill in PC based computer systems.
