The Non-Medical Case Manager role is responsible for providing non-medical case management services specifically aimed at maintaining Ryan White patients within primary medical care systems to enhance health outcomes. This role involves assisting patients in accessing medical, social, community, legal, financial, and other necessary services.

Requirements

  • Minimum of a Bachelor’s Degree in Social Work or related field.
  • Two years’ experience providing case management in a health care setting.
  • Good understanding of working with indigent populations.
  • Must have own transportation and auto insurance.
  • Current Texas State licensure.

Responsibilities

  • Completes client intakes, needs assessments, and care plans upon initial contact with new clients.
  • Assists patients with PAP applications, ADAP, and all other available resources for their service needs.
  • Provides information and assistance with linkage to medical case management and psycho-social services as needed.
  • Maintains regular contact with all clients and completes Needs Assessments and Care Plans with clients, aiming for client self-sufficiency.
  • Helps and empowers clients to develop and utilize independent living skills and strategies.
  • Advocates on behalf of clients to decrease service gaps and remove barriers to services.
  • Provides resource contact information, coordinates, and completes referrals for services needed by clients.
  • Coordinates with community partner agencies to ensure referred services are provided in a timely manner.
  • Documents all client contacts daily in client EMR chart and Take Charge Texas (TCT), and/or Ryan White database, along with notes of care plan.
  • Evaluates patient’s Ryan White Eligibility, including Dental Referrals received by other agencies.
  • Makes information regarding services offered available to the public and uses necessary strategies to identify potential patients.
  • Explains program services, policies, and procedures to patients and staff.
  • Coordinates and assists with Medical Outreach events in the community.
  • Abides by infection control and hazardous materials policies and procedures as dictated in the Centro Del Barrio Infection Control Manual.
  • Participates in the agency’s Quality Improvement Program, including assisting in audit preparation.
  • Abides by all safety policies as dictated in the Employee Safety Manual.
  • Performs other duties as required by the position.
