About The Position

The Friends For All Corporation Bilingual Housing Medical Case Manager will provide direct case management services to supported clients. A Bilingual Housing Medical Case Manager (HMCM) plays an important role in the continuum of HIV care by bridging housing with medical care and other services. Medical Case Management includes the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments as well as other support services to including linkage to health care, psychosocial and mental health support, insurance coverage, educational resources and support, maintenance or establishment of stable housing, and other relevant supportive services. This role is key in ensuring clients in our housing programs remain or become permanently housed by accessing appropriate support services.

Requirements

  • A Bachelor level degree in a health or human services related discipline with equivalent to two year of full time professional case management in a public service agency.
  • A Bachelor level degree in Social Work with equivalent to two years full time professional case management in a public service agency (an appropriately supervised BSW internship may count as one year’s experience)
  • Experience working with persons with HIV/AIDS preferred.
  • Good strategic/critical thinker, self-starter, goal-oriented
  • Excellent verbal and written communications.
  • Ability to operate with purpose, urgency and accuracy in a fast-paced, deadline-driven environment -- with particular attention to detail and organization

  • Maintain confidential information and adhere to all Friends For All policies and procedures regarding sharing of financial information and Protected Health Information (PHI).
  • Must have a valid State Driver’s License, provide proof of automobile insurance coverage to employer at all times and have an automobile to be used daily.
  • Must be sensitive to the culturally and racially diverse nature of clients, patients, employees, and visitors.

Responsibilities

  • Consistent contact with client to assess the efficacy of the service plan and stability in housing as well as access to supportive services.
  • Ensure client achieves viral suppression within one year of program intake through active and consistent treatment adherence counseling.
  • Maintain ongoing contact with referral sources and follow up to ensure implementation.
  • Coordinate services required to ensure the implementation of the service plan.
  • Track receipt of referrals for all persons supported being sure to update client on referral status.
  • Provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
  • Make appropriate referrals for Friends for All services for all persons supported.
  • At least monthly contact with clients to discuss medication and medical appointment adherence.
  • Work with clients to reduce deficits in one or more of the following barriers clients may be experiencing: addiction/substance use, mental health disorders, and/or homelessness.
  • Helping clients make informed decisions by acting as advocates regarding treatment options.
  • This position may require fieldwork including visiting client-members’ homes, hospitals, nursing homes, other organizations, etc.
  • This position may require traveling in your personal vehicle within the counties Friends for All serves.
  • Become a Healthcare Marketplace Certified Applications Counselor in order to assist clients in selecting a Marketplace health plan, if needed.
  • Become SOAR certified within six months to assist clients in applying for Social Security benefits.
  • Be trained on the VI-SPDAT to assist clients who are experiencing homelessness be placed on the By Name List.
  • Document client enrollment in Ryan White Eligibility System (RWES).
  • Provide accurate, complete and timely data entry and reporting of all relevant program and consumer data into Athena, Apricot, CAREware, and/or other reporting systems as requested by supervisor.
  • Document client specific activities in written notes.
  • Establish Ryan White/HOPWA eligibility and provide certification/recertification for consumers if eligible.
  • Establish client eligibility for health insurance or medical coverage and insurance assistance programs as available.
  • Maintain all files in accordance with funding source and agency standards.
  • Initial assessment of service needs includes housing and medical services.
  • Develop a comprehensive service plan within 30 calendar days of assessment that assists the client in maintaining or establishing safe and secure housing as well as access to medical and support services.
  • Periodic re-evaluation and adaptation of the plan should be implemented over the life of the consumer prior to annual certifications.
  • A housing care plan will be developed and updated periodically to address client needs for all clients assigned to a housing program.
  • Ryan White Certifications are to be completed annually.
  • Clients who are in the Scattered Sites/Master Leasing program and are not Ryan White certified will be certified by the Housing Medical Case Manager.
  • When a person has completed the eligibility and enrollment process into the Scattered Sites/Master Leasing Program, they will be assigned a Housing Medical Case Manager.
  • The Housing Medical Case Manager will complete the annual HMIS Program Status Form.
  • Client can be notified within forty-five (45) of the expiration date to complete the form.
  • This will happen annually until the client exits the program.
  • All required HMIS forms are in Apricot.
  • All clients will be excited from the program by the Property Manager.
  • Recruit clients for 340B pharmacy program.
  • Ensure enrolled clients meet program eligibility requirements.
  • Process and follow-up on all program referrals.
  • Complete and track client enrollment in the program.
  • Review and reconcile monthly account settlements by the 15th business day of each month.
  • Review refill analysis and ensure clients have received proper medication as listed on medication list and entered into CAREWare.
  • Maintain up to date Memorandum of Understanding (MOU) for all enrolled clients’ medical providers.
  • Serve on 340B Oversight Committee
  • Other duties and responsibilities as assigned.
  • Participate in weekly or biweekly coaching sessions with supervisor.
  • Return all calls, emails and text in a timely manner.
  • Complete Performance Reviews and SMART GOALS
  • Participate in All Staff Meetings, In-Service Days and Mandatory Trainings
  • May need to travel in personal vehicle to meet staff, clients, or community partners at FFA locations or at other locations to meet the needs of the position
