About The Position

This role focuses on managing complex nephrology care across clinic, dialysis, and inpatient settings to ensure patients receive clear plans and have their questions answered. The Physician Assistant will practice autonomously alongside nephrologists, focusing on critical decision-making rather than just task completion. The position emphasizes proactive patient management, timely documentation, and effective collaboration within a multidisciplinary team.

Requirements

  • Graduate of an accredited PA program and NCCPA certification.
  • Active state license and DEA where prescribing is required.
  • At least two years as a PA in nephrology, inpatient medicine, or dialysis, or a structured nephrology fellowship.
  • Demonstrated ability to independently manage CKD and acute kidney injury.
  • Ability to run a full clinic or rounding list and write complete notes the same day.
  • Proficiency in adjusting antihypertensives, diuretics, binders, and anemia therapy within protocol.
  • Fluency with dialysis access problems, electrolyte emergencies, and renal replacement therapy management.
  • Clear, direct communication with patients, including those who are scared, tired, or non-English speaking.
  • Reliable collaboration with dialysis staff.
  • Comfort with EHR orders, results tracking, and quality metrics.
  • No pattern of incomplete follow-up.

Responsibilities

  • Manage a defined panel of CKD, dialysis, transplant-follow-up, and electrolyte patients, ensuring visit plans, labs, and medication changes are completed before patient departure.
  • See new and return patients independently, presenting only decisions requiring physician input, and completing same-day notes to support billing and continuity.
  • Round on assigned inpatients or dialysis shifts, stabilize acute issues, and hand off a prioritized plan for the next clinician.
  • Reduce avoidable readmissions and missed dialysis complications by escalating fluid, access, and infection problems early and documenting the trigger and response.
  • Own results follow-up, ensuring abnormal labs, imaging, and consults are reviewed, communicated, and acted upon within practice standards.
  • Educate patients and families on the rationale behind diet, medications, access care, and transplant or dialysis choices to improve adherence and informed consent.
  • Partner with nurses, dietitians, social work, and dialysis units to identify and resolve bottlenecks in scheduling, prior authorization, or handoff processes, and measure the effectiveness of these improvements.
