About The Position

Works collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients ensuring that safety, best practice, and high quality standards of care are maintained across the continuum.

Requirements

  • High School Diploma or Equivalent (Required)
  • BLS American Heart E-Card (BLS AHA ECARD) - American Heart Association (AMERICAN HEART), BLS Cert American Heart_non ecard (BLS AHA) - American Heart Association (AMERICAN HEART), BLS Certification Grace (BLS GRACE) - Employee Grace Period for Essential Credential (GRACE), BLS Cert Red Cross (BLS RC) - Red Cross (RED CROSS)
  • One (1) year of experience in a healthcare setting required.
  • Knowledgeable in Excel, Word and PowerPoint and able to use electronic medical records such as EPIC.

  • Success will be measured by the results of the process and outcome performance measures of the population of patients.
  • Requires critical thinking skills, effective communications skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Responsibilities

  • Coordinates a wide range of activities and education to support the patient population by utilizing a variety of communication methods including but not limited to telehealth, face-to-face, text, telephone, or e-mail.
  • May assist with obtaining authorizations and scheduling where appropriate.
  • Utilizes the appropriate reports to prioritize outreach to ensure care coordination.
  • Extract data from medical record sources to gather information and perform outreach.
  • Communicates details to the appropriate team, providers, and/or supervisors.
  • Works closely with physicians and the multidisciplinary care management teams.
  • Facilitates care coordination, and documents efforts.
