About The Position

This is a nationwide, travel-based, per-diem position where the surveyor will be responsible for conducting, planning, organizing, and coordinating healthcare accreditation and/or certification surveys for home health or hospice agencies. These visits must be performed in accordance with the CHAP Standards of Excellence, in strict confidentiality unless otherwise required by law, and within compliance of the policies and procedures that govern the accreditation/certification process. The role requires expert knowledge of CMS Home Health or Hospice Conditions of Participation and the survey process, strong attention to detail, ability to deliver accurate documentation, and the capacity to work autonomously, apply critical thinking, and be adaptable to change.

Requirements

• Bachelor's degree in a related health specialty area.
• Active Registered Nursing (RN) license.
• A minimum of 5 years' experience in compliance or regulatory-related role in home health or hospice industry.
• At least 1 year of experience in a leadership role (responsible for leading a team/direct reports).
• Expert level knowledge of the CMS Home Health or Hospice Conditions of Participation and resources related to the CoPs and the survey process.
• Strong attention to detail.
• Ability to deliver accurate, complete, and well-written documentation.
• Ability to work autonomously.
• Ability to apply critical thinking.
• Adaptability to change.
• A minimum of 10 survey days of availability per month (required travel days are not included in the 10 survey days).
• Availability for 4-6 consecutive travel weeks during the first two months of employment for training (preceptorship).

Responsibilities

• Conducting, planning, organizing, and coordinating healthcare accreditation and/or certification surveys in the community setting.
• Performing visits in accordance with CHAP Standards of Excellence.
• Maintaining strict confidentiality unless otherwise required by law.
• Adhering to policies and procedures governing the accreditation/certification process.
