About The Position

We are seeking a compassionate Registered Nurse (RN) for a full-time Hospice Case Manager position. This role involves providing skilled nursing care to hospice patients in their homes while working independently in the field as part of a supportive interdisciplinary team.

Requirements

• Active New York Registered Nurse (RN) License
• At least 2 years of RN patient care experience
• Valid driver's license and reliable transportation
• Comfortable working independently in the field
• Authorized to work in the United States. No employer-sponsored work visas available
