About The Position

We are seeking a dedicated Registered Nurse (RN) to join our Home Health & Hospice team, providing personalized, high-quality care to patients in the comfort of their own homes. In this role, you will assess, plan, coordinate, implement, and evaluate individualized care for patients across the lifespan, including pediatric, adolescent, adult, and geriatric populations. Requirements

  • Graduate of an accredited school of nursing with a current licensure as a Registered Nurse in the State of Nebraska
  • Valid driver’s license
  • Basic Life Support or ability to obtain within probationary period
  • Interpersonal skills requiring courtesy and tact are necessary to communicate effectively
  • Medium strength with lifting of 50 lbs. occasionally, 25lbs. frequently, and 10 lbs. constantly (Lifting over 35 lbs. requires assistive devices be used)
  • Requires full range of body motion with frequent standing, walking, bending, stooping, and reaching
  • Ability to properly lift position and transport patients (with assistance as defined)
  • Ability to change from one duty to another frequently
  • Analytical skills to organize and prioritize workload to meet patient needs
  • Ability to stabilize unsteady patient when ambulating
  • Ability to record information accurately and follow instructions exactly

Nice To Haves

  • Bachelor of Science in Nursing preferred
  • Minimum 1 year experience in home health, med/surg or critical care area preferred

Responsibilities

  • Assesses patient’s total needs and evaluates home situation
  • Arranges appropriate home care for patient’s nursing needs
  • Coordinates interdisciplinary team planning for each patient’s goals; develops and completes a written care plan
  • Participates in implementing planned changes and activities to improve nursing care
  • Administers skilled nursing care, preventive and therapeutic treatments in the home setting
  • Evaluates personal goals, revises, and formulates new objectives as necessary
