Requirements
- Bachelor’s degree or equivalency in Music Therapy. Masters’ degree preferred.
- Licensed to practice as a Music Therapist – Board Certified (MT-BC) within the state.
- Demonstrates exceptional verbal and written communication, and organization skills.
- Understands hospice philosophy, focus of care is comfort vs. rehab and to maintain care in their residence as long as possible.
- Must have a valid driver’s license for the state providing service within.
Nice To Haves
- Masters’ degree preferred.
Responsibilities
- Assesses music therapy needs of the patient/family and provides music therapy to the hospice patients and their families, as appropriate.
- Consults with interdisciplinary team about patient referrals to assess needs and prioritize intervention.
- Document all care provided on the patient’s clinical records in accordance with documentation requirement
- Educate hospice staff and community about the role of Music Therapist and the use of music therapy interventions with hospice patients and family.
- Provide bereavement follow-up to family members when determined appropriate by the interdisciplinary team.
- Adhere to the practice of confidentiality regarding patients, families, staff and the organization
- Serve as a music enhancement resource to staff
- Support and promote Hospice’s mission to lead in providing exceptional patient – centered end of life care
- Participate in Hospice’s success by ensuring access to our exceptional care
- Timely submission of all documentation
- Performance of other duties as required
- Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
