About The Position

Trillium Health Partners is one of Canada’s largest community-based teaching hospitals, serving the growing and diverse populations of Mississauga, West Toronto, and surrounding communities. The Moir Centre for Complex Continuing Care (CCC) is a 39-bed unit providing care for patients who are medically stable but have complex health needs requiring specialized or technology-based care. Patients admitted to CCC must have care needs that cannot be met in the community or long-term care facilities. The goal of CCC is to maintain or slow the further loss of function, with the team working closely with patients and caregivers to promote wellness, improve quality of life, and achieve collaborative goals of care. The CCC team is interprofessional and offers maintenance therapy to optimize overall health and restore maximum independence, with a goal of transition to other settings whenever possible. Music therapy is a discipline where Certified Music Therapists (MTAs) use music purposefully within therapeutic relationships to support development, health, and well-being. MTAs use music safely and ethically to address human needs in cognitive, communicative, emotional, musical, physical, social, and spiritual domains. Services include the application of music therapy interventions within a therapeutic relationship developed through music-based, verbal, and/or non-verbal communication.

Requirements

  • Member in good standing with the Canadian Association for Music Therapy (CAMT)
  • Successful completion of a minimum bachelor of music therapy degree at a CAMT recognized institution
  • Completion of a 1000-hour clinical internship under the supervision of an accredited Music Therapist (MTA)
  • Experience in provision of Music Therapy programs for adults
  • Demonstrated leadership skills and evidence of continuing education required
  • Experience in program development, including adapting therapeutic modalities to support medically complex patients, including individuals with cognitive and physical disabilities, and mental health needs
  • Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high performance interprofessional team

Nice To Haves

  • Master’s of Music Therapy is preferred
  • Current registration (or qualifying member) with the College of Registered Psychotherapists of Ontario preferred
  • Gentle Persuasive Approach (GPA) certification preferred

Responsibilities

  • Participate, as part of the interprofessional team, in goal setting, evaluation and discharge planning
  • Use appropriate assessment skills to help establish a plan of action by prioritizing and setting goals of treatment in conjunction with patient
  • Provide education, and make recommendations to appropriate community programs/services in conjunction with patient to set an effective discharge plan
  • Obtain and document informed consent for assessment and treatment by a Music Therapist
  • Communicate patient assessment findings, treatment program, and progress with other health care professionals involved in the patient’s care and share responsibility for development and implementation of interprofessional care where indicated
  • Participate in weekly interprofessional progress rounds, monthly professional practice meetings, business meetings and program meetings, as schedule allows
  • Participate in the co-ordination of patient treatment and communicate with other community health care professionals within the “circle of care”
  • Assist and participate in developing, implementing and practicing quality improvement within the program/unit
  • Liaises with external care providers to ensure care needs are met and safety is maintained
  • Maintain and research community partnerships with community programs/services for effective discharge planning
  • Document relevant assessment data, keep current and timely progress and discharge notes for all referred patients while following the professional standards
  • Document required workload measurements data in a timely and accurate manner
  • Assist in the education, supervision and orientation of Music Therapy interns, students and volunteers in the hospital
  • Maintain a focus on continuing professional development by attending and presenting in-house sessions and attending continuing education courses out of hospital and taking part in ongoing reflective practice
