About The Position

Reporting to the Senior Vice President, Patient Care Services and working in close partnership with the Chief & Medical Director for Rehabilitation and CCC, the successful candidate will play a key role in building and sustaining excellence at Trillium Health Partners (THP) across over 400 current Rehab, CCC and Transitional beds. The Director will ensure the delivery of high-quality care for patients and their families across multiple inpatient sites, while developing the future vision for the new, purpose-built 350-bed Gilgan Family Queensway Health Centre, set to open in 2029.

Requirements

  • A background in a regulated health profession with a relevant Master’s degree preferred
  • Progressive leadership with a minimum of 5 years’ experience including leading large-scale clinical programs and/or transformational change initiatives within a clinical setting. Experience at a Director level would be an asset.
  • Inspirational and results-driven leader who has the proven ability to develop and lead strong interprofessional teams, build trust-based relationships that bring all professionals together to build patient-care capacity
  • Well-developed business management skills, including program development and evaluation, workforce planning and management, budget management, and using data to inform decision-making
  • An appreciation of and commitment to a patient-centered and caregiver-included model of rehabilitative care, with a strong passion for implementing new ways of doing things
  • Applicants must be eligible to work in Canada.

Nice To Haves

  • Experience at a Director level would be an asset.
  • Clinical experience in the areas of rehabilitation or transitional care is an asset

Responsibilities

  • Contribute to and lead towards a new vision for Rehabilitative and Continuing Care services at THP, as the Program embarks on significant operational readiness activities in advance of the Gilgan Family Queensway Health Centre
  • With a focus on clinical excellence and evidence-based outcomes, identify, develop, and implement patient and family-centered care initiatives that help build a sustainable, integrated system of rehabilitative and restorative care
  • Lead the advancement of rehabilitative and restorative care services that integrates with and support flow across the broader health service continuum, innovating service delivery to achieve the Quintuple Aim
  • Develop and nurture internal and external partnerships, driving towards continued integration, program excellence and system innovation
  • Build and improve the team by encouraging professional and leadership development, strengthening accountability and improving outcomes
  • Lead several priority projects that are designed to reduce hospitalization and improve system sustainability
  • Support and advance THP’s academic and research mandate, seeking out inter-professional collaboration opportunities by emphasizing the clinical/scientific/research interface
