About The Position

To provide Social Services under the supervision of a licensed social worker, licensed clinical social worker, or a registered nurse in accordance with acceptable social service practices and hospital policies and procedures. Assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Works in close collaboration with members of the healthcare team to provide services and optimize patient functioning in the most efficient and effective manner. Performs comprehensive evaluation including social, emotional, cultural, mental status, environments, and financial circumstances in conjunction with interdisciplinary assessment, and recommends a plan of intervention based on mutually established patient goals. Must be able to demonstrate knowledge of growth and development to coordinate patient care appropriate to the age of patients served on his/her assigned clinical area. Employee must be able to successfully complete job specific orientation Social Services specific competencies and meet/exceed standards as identified in the criteria based on performance appraisal.

Requirements

  • MSW/BSW degree
  • Supervision by a licensed social worker, licensed clinical social worker, or a registered nurse.
  • Adherence to acceptable social service practices and hospital policies and procedures.
  • Knowledge of growth and development to coordinate patient care appropriate to the age of patients served.
  • Ability to successfully complete job specific orientation.
  • Ability to successfully complete Social Services specific competencies.
  • Ability to meet/exceed standards as identified in the criteria based on performance appraisal.
