The MOUD Care Coordinator is responsible for engaging and supporting patients receiving Medication for Opioid Use Disorder Treatment (MOUD) in the Substance Use Disorder Department. Provides skilled nursing services to opioid dependent patients and their significant support systems. Works with the patient and family, provider, and health care team to coordinate care resources and services to improve the quality and effectiveness and decrease the cost of patient care. Demonstrates comprehensive knowledge of community resources and functions as a liaison to facilitate patient access. Collaborates and communicates with stakeholders regarding improvements and the success of the MOUD Program.
