Requirements

  • Bachelor’s level degree in related health care field and two years’ experience in a healthcare field preferred.
  • Extensive health experience may be substituted for the degree requirements.

Responsibilities

  • Works closely with all school administration, school social workers, school nurses and other support staff to coordinate care.
  • Communicates with a parents/student via phone or in- person to coordinate care and obtain completed enrollment forms/consent.
  • Performs data collection pertaining to SBHC and other administrative functions.
  • Maintains registration lists, updates spreadsheets and demographic information, obtains consents and Ferpa related releases.
  • Coordinates with IC primary care for referral and other patient/administrative needs.
  • In charge of Scheduling and check in process of new patients and scheduling follow up appointments for existing patients.
  • Collaborates with all inter-disciplinaries for continuity of medical/behavioral care.
  • Answers phone calls, creates electronic medical record for all new patients, scans all information into primary care and behavioral health medical record.
  • All agency staff are required to attend all mandatory department/agency meetings and trainings.
