About The Position

Metropolitan Area Agency on Aging, a nonprofit in Minneapolis, is seeking to hire a full-time, 40-hour-a-week Minnesota Aging Pathways Resource Coordinator. This position provides person-centered support to older adults and caregivers by completing in-person and phone assessments, offering options counseling on long-term care services, benefits, housing, and financial resources, and assisting individuals and families with care planning and transitions following hospital, rehabilitation, or nursing facility stays. The role involves coordinating services and referrals with healthcare providers and community agencies, advocating for clients to ensure access to appropriate supports, and managing an ongoing caseload with accurate documentation of assessments, plans, and services to promote successful community living. Requirements

  • Bachelor's degree in social work, gerontology, nursing, or a related human service field
  • 2 years of professional experience with case management, care coordination, and discharge planning in a long-term care, hospital, or community setting
  • Knowledge of aging services, community resources, and long-term supports
  • Reliable transportation
  • Heart for working with the elderly

  • Licensed Social Workers preferred

Responsibilities
