Ohana Pacific Health is seeking passionate Minimum Data Set (MDS) Coordinators to ensure the timely completion of accurate MDS assessments, care plans, and electronic MDS transmission for residents. The MDS team coordinates the Interdisciplinary Team (IDT) in the development of the RAI process in accordance with federal and state regulations. Responsibilities include reviewing, editing, and correcting assessments to ensure completeness and accuracy of MDS, CAA's, Care Plans, and care conference notes. The role also focuses on maximizing reimbursement and resident-centered quality of care delivery through accurate capture of resident acuity and documentation of services provided via accurate MDS review.
