Ohana Pacific Health is seeking passionate Minimum Data Set (MDS) Coordinators who will ensure the timely completion of accurate MDS assessments, care plans, and electronic MDS transmission for residents. The MDS team coordinates the Interdisciplinary Team (IDT) in the development of the RAI process in accordance with federal and state regulations. This role involves reviewing, editing, and correcting assessments to ensure they are signed appropriately, signifying completeness and accuracy of all MDS, CAA's, Care Plans, and care conference notes. The position aims to maximize reimbursement and resident-centered quality of care delivery by accurately capturing resident acuity and documenting services provided through thorough MDS review.
