The MFP Transition Manager oversees the MFP Transition Coordination process for individuals who are residing in a Skilled Nursing Facility (SNF) and meet criteria to be a part of the MFP program. Transition Coordinators lead participants and the planning team through a planning process and ensures that participants have supports in place to live successfully in the community. The MFP Transition Manager ensures the effective implementation of the transition services. This role requires a deep understanding of person-centered planning, Medicaid, and community-based services. The Manager will supervise the transition process, ensuring coordination and collaboration among various stakeholders and uphold the quality of transition services. Strong collaboration with other community partners is essential for quality transition planning. This Manager must be able to independently identify, implement and assess system and organizational priorities relative to Olmstead Initiatives. The Manager will demonstrate timely responsiveness to team members, internal LME/MCO staff, members, families, and external partners; communicate clearly and concisely through both verbal and writing skills and case formulation. Additionally, this Manager must demonstrate strong interpersonal skills, and the ability to make prompt independent decisions based upon relevant facts and established processes. Problem solving, negotiation and conflict resolution skills are essential to balance the needs of both internal and external customers. The manager will serve as primary point of contact for general communications between the Department and Transition Coordinators. In addition to supervising Transition Coordinators, the manager will ensure adequate oversight to ensure statewide coverage. The manager will review transition tools and reports for accuracy, completeness, and quality. The manager will ensure the delivery of quality transition practices, manage transition startup funds. The manager will serve or designate a representative as MFP’s housing liaison on MFP housing meetings, attend meetings as directed by the Department, serve as the referral source for targeted/key housing applications, serve as the referral source for targeted/key housing applications, and will attend November MFP Roundtable Meeting each calendar year.
