In this role, you will play a vital part in providing essential support and facilitating activities that promote recovery and well-being within our Mental Health services. Your dedication will directly impact the lives of our clients, families, and stakeholders by offering coaching, education, training, and valuable resource support. Reporting to the Program Manager, our selected candidate will assess clients for psychosocial rehabilitation needs and will support their independence and self-determination in both home and community settings. Responsibilities will also include assisting with daily activities like cooking and budgeting, providing transportation, and facilitating community integration. The successful candidate should also maintain detailed progress notes, collaborate with professionals, and monitor clients' well-being, responding to mental health emergencies as needed.
