About The Position

Merakey is seeking a Mental Health Professional to join their Assertive Community Treatment (ACT) Team in Monroe, LA. This multidisciplinary team provides evidence-based services to individuals diagnosed with severe and persistent mental illness. The role involves providing individual and group therapy, creating person-centered treatment plans, reviewing and updating crisis plans, completing psychosocial assessments, and maintaining clinical documentation. The Mental Health Professional will also provide crisis intervention and support.

Requirements

• Must have an active driver's license within the state of Louisiana.
• Willing to work in a community based setting.
• Must have an active PLPC/LMSW within the state of Louisiana.

Responsibilities

• Provide individual therapy to consumers in service.
• Provide group therapy to consumers in service.
• Create the person-centered treatment plan.
• Review and change the individual crisis plan as required.
• Complete psychosocial assessments.
• Maintain thorough and timely clinical documentation such as progress notes, assessments, and interpretative summaries.
• Provide crisis intervention and support.
