Requirements
- Must have a current license to practice as a LCSW, LMSW, LPC or PLPC in the State of Louisiana.
- Strong organizational and communication skills.
- Proficiency in software for discharge planning, patient assessment, and resource coordination.
- Ability to participate effectively in hospital rounds and department meetings.
- Ability to maintain timely and accurate charting.
- Ability to build and nurture positive relationships with healthcare professionals and team members.
- Dedication to assisting patients with financial and insurance resources.
- Dedication to identifying patient needs pre- and post-discharge.
