About The Position

Bourbonnais Terrace is a Specialized Mental Health and Rehab Facility (SMHRF) that provides care to the non-aggressive adult mentally ill/behavioral health population. The facility focuses on recovery and rehabilitation, boasting a low turnover rate and JACHO accreditation. It is distinct from a Skilled Nursing Facility (SNF), as patients are independent, interactive, and do not require heavy lifting. The center implements the award-winning Life Skills Program from Transitional Care Management. The Mental Health Professional (MHP) is responsible for delivering group and individual psychiatric rehabilitative and case management services to adults with a history of multiple psychiatric hospitalizations who require long-term care stabilization. This role involves collaboration with interdisciplinary teams to develop and execute therapeutic services and interventions. The position emphasizes providing compassion and companionship, treating residents as friends and family members.

Requirements

• Bachelor’s Degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology) and a minimum of 1 year of supervised experience in mental health or human services
• OR an unrelated Bachelor's degree and a minimum of 2 years in a mental health profession/role
• OR a high school diploma and 5 or more years in the mental health profession with clinical supervision

Nice To Haves

• Prior experience as a Psychosocial Rehabilitation Services Coordinator (PRSC) or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility
• Experience with Point Click Care (PCC)

Responsibilities

• Provide group psychiatric rehabilitative services to adults with a history of multiple psychiatric hospitalizations and in need of long-term care stabilization
• Provide individual psychiatric rehabilitative services to adults with a history of multiple psychiatric hospitalizations and in need of long-term care stabilization
• Provide case management services to adults with a history of multiple psychiatric hospitalizations and in need of long-term care stabilization
• Participate with interdisciplinary teams in developing therapeutic services and interventions
• Participate with interdisciplinary teams in implementing therapeutic services and interventions
• Provide compassion and companionship to residents
