About The Position

Bourbonnais Terrace is a Specialized Mental Health and Rehab Facility (SMHRF) providing care to non-aggressive adult mentally ill/behavioral health populations in a quiet suburban neighborhood. The center focuses on recovery and rehabilitation, boasts a low turnover rate, and is JACHO accredited. It is not a Skilled Nursing Facility (SNF), as patients are independent and interactive, not requiring lifting. The center utilizes the award-winning Life Skills Program from Transitional Care Management, which also manages several inpatient healthcare centers across Northern IL & Chicagoland, including Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers. The role of a Mental Health Professional (MHP) extends beyond the job description, emphasizing compassion and companionship, treating residents as friends and family, and partnering with patients on their journey to healthy living.

Requirements

  • Bachelor’s Degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology) and have a minimum of 1 year of supervised experience in mental health or human services
  • Unrelated Bachelors degree and a minimum of 2 years in a mental health profession/role
  • High school diploma and 5 or more years in the mental health profession with clinical supervision

Nice To Haves

  • Prior experience as a PRSC or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility is preferred
  • Experience with Point Click Care (PCC) is ideal

Responsibilities

  • Provides group and individual psychiatric rehabilitative and case management services to adults with a history of multiple psychiatric hospitalizations and in need of long-term care stabilization
  • Participate with the interdisciplinary teams in developing and implementing therapeutic services and interventions
