About The Position

The Mental Health First Response Worker will be responsible for conducting detailed community mental health assessments, SBAR notes, and screening individuals. This role involves assessing, screening, referring, and linking individuals to internal CMHA services or other community resources. The worker will also compile collateral information, participate in Case Disposition meetings, and consult with the Mental Health First Response/RAIT Team to prioritize individuals awaiting service. Key functions include communicating and coordinating information with agency staff, attending weekly psychiatry rounds, providing intervention and brief solution-focused therapy for individuals in psychiatric crisis (via phone or in person), responding quickly to urgent situations, and linking individuals with urgent needs to immediate resources such as food, financial assistance, and housing.

Requirements

  • Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W
  • Registered Nurse or Registered Practical Nurse with current registration with the CNO
  • Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W
  • Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F
  • Valid Ontario Driver’s Licence and access to a vehicle are required

Nice To Haves

  • Preference to those with Canadian Certification in Psychiatric/Mental Health Nursing
  • Preference will be given to a qualified candidate who is proficient in both official languages

Responsibilities

  • Conducting and completing the detailed Community Mental Health Assessment and SBAR notes for individuals referred
  • Assessing, screening, referring and linking individuals to internal CMHA services or alternate community resources
  • Compiling and gathering collateral information
  • Participating in Case Disposition meetings as needed to determine outcomes for individuals referred to the Agency
  • Consulting with the Mental Health First Response/RAIT Team to prioritize individuals awaiting service
  • Communicating and coordinating relevant information with Agency staff
  • Attend as needed weekly psychiatry rounds
  • Providing intervention and brief solution focused therapy support to individuals experiencing a psychiatric crisis (intervention may be via telephone or by personal contact)
  • Providing a quick response to urgent situations that arise
  • Linking individuals with urgent needs to immediate resources (ie securing food resources, financial assistance, and housing)
