About The Position

The Mental Health Crisis Case Manager in the Emergency Department at Highland Park Hospital is responsible for conducting crisis clinical assessments for patients seeking treatment. This role involves obtaining initial diagnostic impressions, assessing clinical needs including vital signs and BMI, and reviewing previous documentation. The Case Manager provides treatment referrals consistent with the patient's medical and psychiatric status, identifies potential problems affecting the patient or department, and reports situations immediately. They also oversee follow-up with referral sources, obtain collateral information from families and support systems, and consult with multidisciplinary treatment teams on patient assessment, treatment, and discharge planning. Additionally, this role involves completing admission paperwork, coordinating with families and community agencies, participating in care conferences, facilitating bed management planning with inpatient units, and educating patients and families about illness and disability. The position also requires following up on documentation with local and state agencies as a mandated reporter per HIPAA guidelines.

Requirements

  • Master’s degree or higher in Behavioral Health field
  • Current State of Illinois Registered Nurse or Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), Licensed Marriage and Family Therapist (LMFT), or Current State of Illinois Registered Nurse.
  • Experience with group, individual, and family therapy.
  • Experience with adolescents and families in crisis.
  • Familiarity with Cognitive Behavioral Therapy and Dialectical Behavioral Therapy.
  • Minimum of two years of experience in crisis/emergency services, behavioral health setting preferred.
  • Knowledge of patient rights and law pertaining to mental health
  • Emergency Room mental health therapy

Responsibilities

  • Conducting crisis clinical assessments for patients seeking treatment.
  • Assessing patients' clinical needs, including vital signs, weight, blood alcohol, and BMI calibration for specific populations.
  • Obtaining initial diagnostic impression and confirmation during the disposition process with the physician or designee.
  • Reviewing previous documentation prior to assessment and case formulation.
  • Providing treatment referrals consistent with the customer’s medical and psychiatric status and symptoms.
  • Identifying potential/actual problems that may adversely affect the patient and/or department and reporting situations immediately.
  • Overseeing follow up with referral sources as appropriate.
  • Obtaining collateral with families and support systems through face-to-face interviews and/or phone calls.
  • Consulting with multidisciplinary treatment team members on the assessment, treatment, and discharge planning of patients with mental health problems.
  • Completing and acting as a resource to hospital staff on admission paperwork and patient rights.
  • Contacting and coordinating with families and community agencies as appropriate.
  • Participating in care conferences and emergency department huddles to discuss key social, financial, and discharge planning factors.
  • Facilitating bed management planning and coordination with Endeavour Inpatient Psych units.
  • Educating patients and patient families regarding the effects of illness and disability on personal adjustment, family dynamics, relationships, social issues and the like.
  • Following up on documentation with local and state agencies as a mandated reporter per HIPAA guidelines.
