About The Position

As a Mental Health Counselor at Community Health Center of Buffalo, Inc., you will provide outpatient clinical services in an Article 31 setting, supporting individuals navigating mental health and co-occurring challenges. This role combines meaningful, relationship-based clinical work with a strong interdisciplinary team environment. You will play a key role in helping patients access care, build stability, and move forward in their recovery using trauma-informed, person-centered approaches.

Requirements

  • LMHC, LMSW, or LCSW (NYS permit holders encouraged to apply)
  • 3+ years of clinical experience preferred
  • Experience with trauma-informed care
  • Strong interpersonal, communication, and organizational skills
  • Comfort working in a fast-paced, team-based environment

Nice To Haves

  • EMDR experience is a plus

Responsibilities

  • Conduct comprehensive psychosocial and diagnostic assessments, including risk evaluations
  • Develop and implement individualized, recovery-oriented treatment plans
  • Provide individual therapy using evidence-based approaches such as CBT, DBT, and trauma-informed care
  • Deliver crisis intervention and coordinate with higher levels of care when needed
  • Collaborate with medical providers, care teams, and community partners to support whole-person care
  • Participate in team meetings, case conferences, and clinical supervision
  • Connect patients to community resources including housing, substance use treatment, and social services
  • Maintain timely and compliant clinical documentation in accordance with OMH and Article 31 standards
  • Meet productivity expectations while maintaining quality, patient-centered care
