The Doe Fund is seeking a dedicated Case Manager/Mental Health Counselor for the Crotona House Permanent Supportive Housing Program. This program provides long-term residential care and 24-hour support services for homeless men and women living with HIV/AIDS. The program is designed to maximize residents’ independence while ensuring their physical and emotional security within a supportive community environment. The majority of Crotona Park residents will be referred by the New York City HIV/AIDS Services Administration (HASA) and some by the New York City Housing Authority (NYCHA). The Case Manager/Mental Health Counselor is responsible for delivering supportive case management services to HASA-funded residents. The ideal candidate will demonstrate strong clinical skills, excellent communication abilities, and a deep understanding of the unique needs of individuals living with HIV/AIDS.
