Requirements
- Bachelor’s degree in social work or related discipline (i.e. Health Sciences)
- Minimum three years related experience
- Counselling, crisis prevention and intervention experience
- Experience working with individuals having serious mental illness is essential
- Excellent oral and written communication skills
- The successful applicant will be required to provide a police clearance that meets agency standards
- A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.

- Knowledge and practice of psychosocial rehabilitation approach and advanced clinical and assessment skills are assets
- Second language is considered an asset
- An understanding of cultural diversity is considered an asset
Responsibilities
- Provide comprehensive and collaborative community based case management response to individuals age sixteen and over with serious mental illness and/or addictions within York Region.
- Utilizing a Recovery Model framework, assist service users to identify needs and goals that will enhance the quality of their lives.
